How it works
- Download the form from the Families page.
- Fill it out. Physical forms need to be completed by your child’s doctor.
- Take a photo of each page, or scan it, and upload it here.
Private & secure
Documents go straight to the Children’s World office by email. They aren’t stored on the website. Paper copies are always welcome at the front desk.
